Heart to Heart

Heart to HeartTM is an education and support program for cardiac patients and their families.

Heart to HeartTM is helpful to anyone who has had a heart attack, heart surgery, angina - or is a high risk candidate for heart disease.

Heart to HeartTM is offered as a series of regular sessions. Each session features a video, informal group discussions and activities. All sessions are designed to help make living with heart disease easier for you and your caregiver.

In each 2 hour session, you will learn about how your heart works, heart disease treatments and ways to improve your eating and exercise habits to reduce your risk of further heart disease. You will also have the opportunity to ask questions and to share your concerns. You will be able to learn from others about what has worked for them, and offer your own ideas about dealing with the effects of heart disease.

There are regular sessions of a small group of patients, caregivers and a knowledgeable facilitator. The six sessions include:

  • Meet Each Other and Meet Your Heart
  • Procedures, Treatments, Medications and Plans
  • Eat HeartSmartTM
  • On The Go - Physical Activity
  • Stress Management and the Emotional Journey to Recovery 
  • Community Resources and Wrap-Up

The Heart to Heart program is not intended to replace therapy or treatment. It will provide you with information and support to help you cope.
